Data may be requested by any UNMC faculty, The Nebraska Medical Center physician, or student, with the approval of their mentor, principal investigator or manager. 

Complete the EHR Service Request Form at right.  Assistance with filling out the EHR Research Request form may be requested from the Associate Research Subject Advocate, Deborah Meyer, RN.

Frequently Asked Questions (FAQs)

  1. What constitutes a Feasibility Study?  Feasibility studies are generally limited data retrieval requests used to ascertain whether sufficient patient or sample populations are available to accomplish the proposed research studies.
  2. What is the difference between Identified and De-Identified requests?  De-Identified information has had all Patient Health Information (PHI) removed from the record that could be used to identify the subject.  This includes date elements except year, and medical record numbers.  Questions related to what constitutes PHI may be referred to the Privacy Officer (402-559-6767).
  3. What is an ICD9 Code and where can I find that?  ICD9 codes or International Classification of Diseases are the categories used to describe categories of diseases and health problems widely used in the medical record.  Links to compendiums of ICD9 codes can be found at the right.

Protected Health Information
Protected Health Information (PHI) is information that is very specific to the individual such as birth date, telephone number, and social security number, that the Health Insurance Portability and Accountability Act (HIPAA) was intended to protect, so as to protect individual patient confidentiality.
