Required Information

Must be submitted before application will be considered for acceptance:

1. Letters of recommendations - Three, including at least two from your U.S. postgraduate program faculty

2. Official copies of transcripts from all undergraduate colleges attended

3. Official copy of transcript from dental school and from the U.S. postgraduate program

4. Current catalog from the school where you obtained the dental degree and the where you obtained/are obtaining your postgraduate certificate

5. Dental school and postgraduate curriculum with complete course synopses

6. National Board Scores Part I - Minimum score of 85 required

7. Official DAT scores if you have taken the DAT

8. Description of grading system used in all schools you attended

9. TOEFL test scores;

550 Paper based
213 Computer Based
79-80 Internet Based

Application processing fee of $50.00 is non refundable
