The installation of Adobe Acrobat Reader is necessary to view/print the Human Genetics Laboratory Request Forms. Please follow the instructions below to install Adobe Acrobat Reader free of charge.

Proceed to the forms request area if your workstation has Adobe Acrobat Reader installed.

Adobe Acrobat Reader for Windows '95 Installation Instructions

• Download the Adobe Acrobat Reader software to your computer. 
  To download the software, click on the yellow and red button to the right: 
• When you are asked to save the file, it is best to use a directory or a location that is easy to find
  (example: C: emp). Press the save button to proceed with downloading.
• After the software has downloaded open Windows Explorer and locate the file.
• When you locate the file double click on the name to begin the installation.
• Follow the prompts that you are given selecting the normal or basic install, using the default options.
• The software will ask several questions always answer affirmatively or use the next option.
• The software will install itself when you answer the last question.
• When the software has been installed you can then delete the file that was downloaded.  

After you have downloaded and installed the Adobe Acrobat Reader you can view/print the Human Genetics Laboratory Request Forms listed below. 

After you click on the form name the workstation will ask you if you would like to "save to disk" or "Open it". Please choose "Open it".
