NEED A LIST OF CVS CAREMARK PHARMACIES?

 

Go to www.caremark.com and use "Find a Local Pharmacy" feature.  (You will be required to register as a member on your initial visit to this website);

 

Go to www.nebraska.edu/benefits (you may also reach your University of Nebraska Benefits Website through the Quick Links on the Firefly website).  On this website, click on Health Benefits, and then Prescription Drugs.  At the end of the information on prescription drugs, there is a link to a Nebraska and Iowa Pharmacy Provider Network Search;

 

And, finally, Human Resources is developing a website which will list the network pharmacies and other information.  It will be announced when it is available.

 

 WHAT OTHER ALTERNATIVES DO YOU HAVE FOR FILLING YOUR PRESCRIPTIONS?

 

Use the Nebraska Medical Center Pharmacy on the 2nd Floor Durham Outpatient Center.  Hours are 7 am – 9 pm Monday through Friday and 8:30 – 4:30 on Saturday and Sunday and holidays.  Or, use the Nebraska Medical Center Bellevue Pharmacy.  Please call 402/595-1156 for their location and hours.

 

Utilize the mail prescription service through CVS Caremark.  To switch a current prescription to mail, call 1-800-875-0867.  You will need your CVS Caremark ID card, the names of your medications, your doctor's name and phone number, and your payment information.

 

Use any one of approximately 57,000 CVS Caremark network pharmacies nationwide.

 

 HOW DO I TRANSFER PRESCRIPTIONS TO ANOTHER PHARMACY?

 

It's easy to transfer your prescriptions to another CVS Caremark participating pharmacy.  In many cases your new pharmacy can handle the transfer for you.  Either bring your prescription bottle with the label intact into the new pharmacy with you or, when speaking to the provider on the phone, you will need to give the information on your prescription bottle label to them and they can handle your next refill.

 

 HOW DO I GET ANOTHER CVS CAREMARK ID CARD?

 

You can obtain another paper CVS Caremark ID card by going to the University of Nebraska Benefits Website (www.nebraska.edu/benefits).  You will find the ID card under the link to Benefits Forms.   Your CVS Caremark ID number is a series of zeroes in front of your personnel number to total nine digits (i.e., 000012345).
