Request for Change Instructions: Any change in a previously approved protocol must undergo review and approval prior to implementation of the change. Submission requirements depend on the type of request as follows:
- You are requesting to change, add and/or delete personnel, a Request for Change form is to be used.
- You are requesting to change the title of your protocol, a Request for Change form is to be used.
- You are requesting any change, other than personnel or title changes, a Request for Change form is to be used AND submission of a revised version of entire protocol in which the proposed changes have been incorporated and underlined is required.
Note: A change in protocol other than change in study title or study personnel requires submission of a revised IACUC application that incorporates the changes in all appropriate Sections. Submissions without a revised IACUC application will be returned without review.
