The Geriatric Medicine Clinic is located on the main level of the Home Instead Center For Successful Aging, 730 South 38th Avenue, Omaha, NE. The HICSA building is located on the northwest corner of 38th Avenue and Leavenworth Street. Patient and visitor parking is located at the main entrance, just off 38th Avenue.

Monday through Friday, 8:00 AM - 4:30 PM. After hours, on weekends and holidays, call the main clinic number, (402) 559-9600. A recorded message will instruct you how to contact the physician on-call.

MEET OUR STAFF:

Our staff is comprised of Geriatricians (physicians who are specially trained in the field of Geriatrics), Geriatric Fellows (physicians in training to become Geriatricians), Nurse-Practitioners, Nurse-Care Managers, Medical Social Workers, a Pharmacist, a Medical Assistant, and our Receptionist. PATIENT QUALIFICATIONS:

Minimum age to access our clinical services is 65.

We accept the following types of insurance: Medicare and Medicare Supplement plans; Major Medical Insurance, PPO Insurance and State Medicaid.

Persons who subscribe to a Managed Care Plan or Health Maintenance Organization (such as United Health Care/Secure Horizons) may not be covered by their plan for some medical services through our clinic. Please contact your HMO customer service representative for referral to an "in-network" provider.

PATIENT CARE PROGRAMS:

  • The Geriatric Assessment Program provides a comprehensive evaluation of older persons who are experiencing memory loss and/or a complex of health problems that rob them of their ability to perform essential daily activities. This includes physical, memory and mood evaluations; a review of medications, and an interview with the patient's family or significant support persons. A detailed treatment plan is developed and is then implemented with the patient, the family caregivers and the family physician.

The Division of Neurological Sciences also offers a Memory Disorders Clinic. This clinic evaluates and treats memory loss in otherwise healthy adults of all ages. It also specializes in diagnosing and treating memory loss in persons who suffer symptoms of a neurological disorder (Parkinson's Disease and related movement disorders). The Memory Disorders Clinic is similar to the Geriatric Assessment Program in that it develops and implements a treatment plan with the patient, family members and the patient's primary physician. Unlike the Geriatric Assessment Program, it does not evaluate or treat other physical or mood problems.

  • Primary Medical Care can be provided for older persons who reside in the Omaha area.
  • The Home Care Program provides primary care to a limited number of older persons who live at home, but who are too physically frail to come to our office.
