Medications; When possible, attempt to convert the patient's medications to the least frequent dosing possible. This will reduce medication errors of omission and improve quality of life of the patient with less interruption of their life to take pills. In patients with swallowing disorders, a simple order such as "may crush medications or provide as liquid," will allow the facility to adjust medications to the patient's swallowing abilities. Skilled nursing facilities require every medication to have an indication.  This can be satisfied by putting the diagnoses in parentheses after the drug. Lastly, medications that are commonly forgotten in transition include eyedrops, topicals (e.g Lidoderm patches), mouth rinses and skincare products.
