Bariatric information sessions
Where: The Nebraska Medical Center campus in the Storz Pavilion, Lower level rooms 1-4 which are accessible from Clarkson Tower.
When: the 2nd Thursday of each month - 6 p.m. to 8 p.m.

There are two ways to access the meeting site.
- Park in the Clarkson parking lot located on 42nd and Dewey Streets. Enter into the main lobby of Clarkson Tower. If needed, wheelchairs are available at the front desk. Follow the hallway towards the Storz Pavilion Cafeteria. The elevators can take you to the lower level entrance of the Storz Pavilion.
- On the south side of the Clarkson parking garage there is a drive leading to the lower level of the Storz Pavilion. The entrance will face east and go directly to the conference room where the meeting will be held.
Required Documentation:
You will need to print the following documents, fill them out and bring them to the Bariatric Information Session that you have signed up for.
- Bariatric Questionnaire
- Sleepiness Scale
- Patient Consent and Disclosure for Treatment
- Program Application Form
- a copy of your current insurance card - front and back
- a copy of the past 2 yrs of your medical history
