For Appointments

Check-In/Registration: Upon arrival at the clinic, please check in with the staff at the front reception desk. This will alert them that you are in the waiting area. The receptionist will verify your address, phone number, insurance information and other appropriate information. The receptionist will also request the name and address of your Primary Care Physician (PCP) and/or referring physician for future communication back to these physicians. You may be asked to complete a patient history form or other forms while you are waiting to see your doctor.

Your Visit with Your Doctor and Nurse: Following the check-in/registration at the reception desk, you will be called to an exam room by a medical assistant. The assistant will take your vital signs and weigh you. The assistant will ask you about allergies, the medications you are taking and inform you of your schedule.
Your examination will be completed by a staff physician and/or a nurse practitioner. The staff physician in charge of your care may be assisted by residents. The physicians or nurse practitioners will conduct a medical history and physical and a neurological exam. They will review with you the need for any further medical testing or consultations with other physcians.

During Your First Visit: During your visit, you may receive information about plans for further testing, test results (if available), diagnosis and treatment plans.

Scheduling of Tests/Return Appointments: At the end of your clinic visit, the clinic staff will schedule any laboratory, radiology or electrophysiology tests or other procedures that have been ordered by your physician. Some of these tests may be able to be arranged and completed on the same day as your visit. However, if another day is needed to schedule these procedures/tests, the clinic staff will make every effort to schedule per your convenience. Please direct any questions you may have about the tests/procedures to the nurse or physician.
Follow-up appointments are scheduled at the clinic at the end of your clinic visit. These follow-up clinic visits are scheduled within the indicated return time frame. Efforts are made to complete these return appointments at a time convenient for your schedule. You will receive from the receptionist an appointment card with the return appointment visit date and time as well as clinic phone numbers. Approximately 2-3 days prior to the appointment, you will receive an automated voice reminder of your scheduled appointment.
