Every presenter must complete a Biographical Data form and a Financial Disclosure form. One Documentation form must be completed for EACH presentation. This is to be done by the presenter. When two or more persons are collaborating on one presentation, they should work together to complete one Documentation form. Presenters who are working directly with CNE staff to plan conference logistics may be asked to complete the Audio/Visual form to assure that their audiovisual needs are met. Presenters who need to complete this form will be notified on an individual basis.
