The FHCC is a nurse-managed primary care clinic located in the UNMC Community Health Center at 5155 South 36th Street, Omaha, NE 68107 (402) 595-2296. The College of Nursing received a grant from the U.S. Department of Health and Human Services for nurse practitioner training provided start-up monies for the center.
The mission of the center is to provide quality comprehensive primary health care services, including patient education, patient advocacy and case management to vulnerable individuals, families, and organizations in and/or closely located to South Omaha. The model of care allows nurse practitioners to integrate practice, education, and research in the delivery of high quality, cost-effective health care to the individuals and families.
Services offered include primary health care for adults and children and mental health counseling. Nurse practitioners and undergraduate students, working under the supervision of nursing faculty, provide client care. Undergraduate students may be assigned to work with the nursing coordinator in learning management roles.
The advanced practice, holistic nursing model demonstrated in the center allows clinicians and clients to work collaboratively to address all aspects of health care need and assure quality care and optimal health outcomes. The Family Health Care Center accepts most forms of health insurance as well as Medicare and Medicaid. However, no one is denied care due to an inability to pay. The clinic accommodates the financial capabilities of each patient through a sliding scale to offer primary health care services at lower rates than may be available elsewhere.
The center serves an average of 300-325 people per month. It traditionally serves vulnerable populations who come from a variety of ethnic backgrounds, many of whom are living in poverty and are either under-insured or under-served medically.
