After viewing the entire lecture, follow the link to the evaluation form. To print out the evaluation, Right-Click on the evaluation form which it is in the RealOne Window.
Complete the evaluation form.
The instructions on the evaluation form will ask you to print out a Nursing Registration Form at http://www.unmc.edu/obgyn/videos/nursingreg.htm. Complete this form.
For each lecture viewed, mail the completed Nursing Registration Form, completed lecture evaluation form, and $10 to cover the cost of the 1.0 nursing credit to:
