AHRQ Hospital Survey on Patient Safety Culture (HSOPSC)

The University of Nebraska Medical Center (UNMC) is offering to administer and analyze this survey in small rural hospitals on a fee-for-service basis.

Why is this service being offered?

UNMC has administered this survey in over 56 Critical Access Hospitals, achieving an overall average response rate of 70%. By engaging UNMC to conduct this survey, small rural hospitals obtain the following:

bullet

an unbiased measurement of employee beliefs and behaviors regarding patient safety,

bullet

assurance of employee anonymity as employees use a postage paid envelope to mail surveys to UNMC for data entry and analysis,

bullet

ability to benchmark findings with hospitals of similar size,

bullet

consulting assistance for action planning to respond to strengths and weaknesses identified in the survey,

bullet

electronic copies of all reports and graphics to communicate findings to medical staff, employees, and the board of directors.

What does my organization need to do if we are interested in this service?

Request an estimate from UNMC by contacting Anne Skinner, Health Data Analyst (phone 402-559-8221 or e-mail askinner@unmc.edu). Ms. Skinner will send you an e-mail with an attached Excel worksheet to determine the number of employees to be surveyed. The survey is intended for four groups of employees 1) those with direct patient contact, 2) those whose work directly affects patient care (lab, pharmacy personnel, dietary aide, housekeeping), 3) supervisors, managers, administrators and 4) physicians and mid-level providers. The total cost to administer and interpret the survey is based on fixed and variable costs based on the number of employees to be surveyed. For example, to survey 100 employees the fixed costs are $600 and variable costs are $1,413.02 for a total cost of $2,013.02.

What does my organization need to do if we decide to conduct the survey??

Distribute a pre-notification letter (template provided) to each employee who will receive a survey. This letter can be distributed in-house, with employee pay checks, or by mail to employee homes.

Distribute the first wave of the survey two weeks after the pre-notification letter is distributed. Your organization will receive a box containing envelopes labeled with each employee’s name. Each envelope will contain a cover letter introducing the survey, the survey form, and a postage paid business reply envelope employees can use to mail the survey to UNMC.

Distribute a reminder/thank you post card to each employee two weeks after the first wave of the survey.
 

bullet

Distribute a second envelope to each targeted employee two weeks after the post card reminder. This second envelope will contain a thank you letter to those employees that previously responded to the survey or it will contain a final cover letter, survey, and return envelope if the employee has not already responded.
