When the principal investigator of the grant is at UNMC

In addition to the grant application and appropriate internal forms, SPAdmin will need the following documents for each subcontract prior to submission:

• Intent to Form a Consortium OR Face Page Form Page 1
• Statement of Work
• Detailed budget
• Budget justification
• Checklist or other documentation of F&A requested
• Contact information

When the principal investigator of the grant is at another institution

In addition to the appropriate internal forms, SPAdmin will need the following documents for UNMC's part prior to submission:

• Intent to Form a Consortium OR Face Page Form Page 1
• Statement of Work
• Detailed budget
• Budget justification
• Checklist or other documentation of F&A requested
• Copy of any other documentation the other institution requires; e.g.
• Biographical sketch
• Resources page
