Creating a UNMC Financial Aid Account

UNMC has elected to use Dynamic Forms for the financial aid award process. Dynamic Forms is a secure web site and is not authorized to use your information, in any way, for solicitation.

This account is NOT the same as your PeopleSoft account. If using your Lotus Notes email address and password account information will assist you in remembering this information, you may use it to establish this account.

For those who choose not to e-sign and electronically submit forms, you have the option of printing the document for mailing.

You will be required to create a security question and answer. The security question and answer will be used to validate the user in the event of a forgotten password.

In the event you attempt to retrieve a forgotten username and/or password, you will need to add notify@nelnet.net to your list of acceptable senders and/or contact your ISP to ensure that you receive e-mail from Dynamic forms.

Listed below are the requirements for setting up a login and password account.

1. A valid login must be at least eight (8) characters in length and must begin with a letter and contain at least one (1) digit.
2. An acceptable password must be at least eight (8) characters in length, cannot contain the username, and must contain characters from three of the four following groups:
   1. Lower case letters
   2. Upper case letters
   3. Numbers
   4. Alphanumeric characters or symbols (i.e., @, $, &)

Use your financial aid login and password for each required financial aid process.

To create your login and password account, Log in to Dynamic Forms.  You will be required to enter some information twice.
