Federal Verification Instructions for the Parent

Parents, you will be required to create your own secure on-line account to complete the Student Verification Worksheet.

You must complete your own on-line forms and mail your tax verification documentation directly to our office.  Place the student's name and Social Security Number on each document to insure it is matched to the correct file.

Step 1:  Complete the Parent Verification Worksheet (on-line filing) OR (.pdf document)

Step 2:  Request an IRS Tax Transcript

Due to changes in the Federal Verification Regulations, students must request and submit an IRS Transcript in lieu of copies of their Federal Income Tax Form 1040, 1040A, or 1040EZ.
Federal Income Tax Forms 1040, 1040A, and 1040EZ are no longer accepted. Also, State Tax Returns are not acceptable. 

NOTE:  THE IRS ACCOUNT TRANSCRIPT IS NOT THE SAME AS AN IRS TAX TRANSCRIPT, NOR CAN IT BE ACCEPTED BY THE OFFICE OF FINANCIAL AID.  PLEASE REQUEST THE IRS TAX TRANSCIPT ONLY!

IRS Transcripts can be requested THREE (3) ways:
     1.  On-line request at IRS.GOV. 

     2.  Complete and MAIL Form 4506-T - Request for Transcript of Tax Return to the following address:  OR

    3.  Complete and FAX From 4506-T - Request for Transcript of Tax Return to 801-620-6922 (see page two (2) of instructions)

Students have 60 days to complete the verification process. If you experience delays in obtaining IRS transcripts, please notify our office by email. We will make a note in  your file.  As a courtesy, we will continue to send you a reminder letter every 30 days until your file is complete. File completion delays as a result of IRS processing will not result in reduced financial aid packages for those students who completed their FAFSA before the deadline.  

UNMC reserves the right to collect additional information from you and/or your parents should the verification process result in questions regarding your financial aid application.
