Federal Verification Instructions for the Student
Students, you will be required to create your own secure on-line account to complete the Student Verification Worksheet.
2011/2012 Academic Year

You are required to mail copies of Federal 1040 U.S. Tax Returns, W2s, and any other documents detailed on the verification worksheets. State Tax Returns are not acceptable.
PDF download and supporting documents (i.e. Federal Income Tax Forms, W2s...etc) need to be printed, completed, and mailed to the UNMC Office of Financial Aid. (address below).
2012/2013 Academic Year

Due to changes in the Federal Verification Regulations for the 2012/2013 academic year, students must request and submit an IRS Transcript in lieu of copies of their 2011 Federal Income Tax Form 1040, 1040A, or 1040EZ. Federal Income Tax Forms 1040, 1040A, and 1040EZ are not longer accepted. Also, State Tax Returns are not acceptable.
IRS Transcripts can be requested two (2) ways:
1. On-line request at IRS.GOV, or
2. Complete and mail Form 4506-T - Request for Transcript of Tax Return to the following address:

Students have 60 days to complete the verification process. Failure to complete the process will result in your ineligibility for federal financial aid for the academic year.
UNMC reserves the right to collect additional information from you and/or your parents should the verification process result in questions regarding your financial aid application.
