Rhinoplasty is a surgical procedure to change the size and/or shape of the nose. It is one of the most commonly performed plastic surgical procedures. The appearance of the nose can be changed by surgically manipulating the structures underneath the skin, specifically the nasal bones and cartilage. The procedure is not intended to dramatically change the patient's appearance but rather to enhance the facial appearance as well as creating a nose that is in more harmony with the individual's face. The type of rhinoplasty performed will depend on the individual characteristics of the patient's nose and what that person's desires are for nasal appearance. The surgery can be performed "open" or "closed". An open rhinoplasty is often performed on patients who need significant modification of the tip cartilages. A small incision is made on the columella and inside the nose as depicted in the diagram below. The incision on the columella heals very well and is virtually imperceptible. A closed rhinoplasty is more commonly performed when the primary goal of the rhinoplasty is to address bone abnormalities such as a dorsal hump. The procedure can be performed under general anesthesia or local anesthesia with intravenous sedation. If the patient considering rhinoplasty also has nasal airway problems such as a deviated nasal septum, the septal deviation can be corrected at the time of the surgical procedure.
After the surgical procedure a nasal splint will be placed on the nose that usually remains for seven to ten days. The splint helps the bones and cartilage maintain their new shape.
Complications of rhinoplasty include bleeding, infection, nasal airway obstruction, nasal deformity, scarring as well as reaction to the anesthesia. Minor bleeding is not uncommon after the procedure and should be expected. Not infrequently minor "touch-up" or revision surgery is necessary after the healing has completed. The risks of surgery are low in healthy individuals and when performed by qualified plastic surgeons.
