2022 - 2023 RSS Application
- Your 2022-2023 RSS Application must be submitted by May 20, 2022 to allow time for processing.
- The new fiscal year starts July 1, 2022.
- The Course Director must complete the section of the application that focuses on needs, gaps, and objectives.
- Conflict of Interest forms must be submitted with the application.
2022 - 2023 Non-Credit RSS Application
- Your 2022-2023 Non-Credit RSS Application must be submitted by May 20, 2022 to allow time for processing.
- The new fiscal year starts July 1, 2022.
- Because these RSS Activities will not be accredited for CMEs, Conflict of Interest forms are not required to be submitted with the application.
Conflict of Interest Form
All presenters and planning committee members must provide a current COI PRIOR to the event.
Resolution of Conflict of Interest
This form must be completed by the Course Director PRIOR to the event if a presenter or planning committee member discloses financial relationship with an ineligible company.
RSS Activity Description Form
This form must be completed by a lecture-based activity speaker to indicate his/her demographics, presentation title, and objectives.
RSS Patient-Based Monthly Report
Visiting Faculty Funding Form
This form must be completed by the RSS Coordinator for visiting speakers.
Accreditation and Credit Designation Statement
All announcements should have the following accreditation and credit designation statement – verbatim, punctuated and formatted exactly as shown below.
The second paragraph should include the correct number of credit(s) where indicated.
