Throughout this pandemic, UNMC has focused on the evolving science to keep our campuses and the communities that we serve functional and safe. As a result, UNMC will now require all students, staff and faculty to report they are fully vaccinated against COVID-19 by Oct. 22, 2021.

UNMC is distributing unique ID badge stickers to individuals who are fully vaccinated and have their COVID-19 vaccination recorded in the Health Tracking System.

Once your vaccination status has been recorded in the Health Tracking System, your sticker will be automatically mailed to your home address. UNMC will begin to mail out stickers in mid- to late August and please allow 3-4 weeks for mailing.

If your address is correct and you have updated your COVID-19 vaccination records, you will need to request a new sticker to be mailed to you using the replacement request form. Please allow 3-4 weeks for mailing.

If you are a new student or employee who is fully vaccinated, please log into the Health Tracking System to record your vaccination status. Your sticker will be automatically mailed to you at your home address. Please allow 3-4 weeks for mailing.

For all people who fall under the "Non Employee or Volunteer" category (including Emeritus Professors) you can use this link to request a sticker.  You will be asked to fill in your contact information and role at UNMC.  Once you complete the form you will be given specific instructions on where and when to pick up your sticker for your UNMC ID badge.
