Examples of reports would include generating a list of patients with particular diagnosis codes or generating a list of patients with a particular procedure code.<\/p>\n<p><b>Answer:<\/b><\/p>\n<p>First you must verify that the disclosure is permissible under HIPAA.  Refer to the Use and Disclosure of PHI policy (UNMC Policy #6057 <a href=\"https:\/\/info.unmc.edu\/policy\/PHI.htm\" target=\"_blanK\" rel=\"noopener\">Use and Disclosure of PHI)<\/a>.  Ask the requestor to complete a &#8220;Request for PHI&#8221; form <a href=\"https:\/\/info.unmc.edu\/policy\/ExhibitIII_6057.doc\" target=\"_blanK\" rel=\"noopener\">(Request for PHI form)<\/a> and submit it to you prior to releasing the information. <\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.unmc.edu\/newsroom\/wp-content\/uploads\/2022\/08\/HIPAAbox.jpg\" border=\"0\" align=\"left\" alt=\"picture disc.\"><\/p>\n<p>If you&#8217;re a manager, please ensure all of your employees are informed of the contents of these messages and how it applies to your work area.
