how the CVS Caremark mail order Service Program Works:<br \/>\n<UL><LI>Log in to the <a href=\"http:\/\/www.nebraska.edu\/benefits\" target=\"_blanK\" rel=\"noopener\">University&#8217;s Benefits Web site<\/a><br \/>\n<LI>On the right-hand side of the page click on: Benefit Forms &#8211;&gt;<br \/>\n<LI>In middle of the page you will find: Prescription Drug Program &#8211;&gt; Caremark Mail Order Request Envelope<br \/>\n<LI>Print this two page form, complete,and mail it to the CVS Caremark address below (address also is typed address in the upper right-hand corner of the first page of the form):<br \/>\n<BR>CVS CAREMARK<br \/>\n<BR>PO BOX 94467<br \/>\n<BR>PALATINE, IL 60094-4467<\/UL><\/p>\n<p><TABLE border=\"0\" width=\"260\" cellspacing=\"5\" cellpadding=\"3\" align=\"right\"><br \/>\n<TR><br \/>\n<TD><br \/>\n<TABLE border=\"1\" width=\"250\" cellspacing=\"1\" cellpadding=\"5\" bgcolor=\"E8BC5A\"><br \/>\n<TR><br \/>\n<TD><\/p>\n<p align=\"left\">\n<font face=\"arial\"><br program.<\/p>\n<p>Contact the benefits office at 559-4340 for more information.<br \/>\n<\/P><br \/>\n<\/FONT><br \/>\n<\/TD><br \/>\n<\/TR><br \/>\n<\/TABLE><br \/>\n<\/TD><br \/>\n<\/TR><br \/>\n<\/TABLE>If you are accessing the program for the first time, you will need to enclose the original prescription(s) from your doctor for all requested medications and the required payment of the deductible and co-payment if filling a brand name drug (or give them credit card information).  <\/p>\n<p>If you are filling a generic, you will need to enclose a note that indicates that as of Jan. 1, no co-payment is required.<\/p>\n<p>Again, this free generic benefit only pertains to those who completed the Wellstream\/Health Risk Assessment during the benefits enrollment period of Nov.
