The names of your covered dependents will not be printed on the cards.  <\/p>\n<p>You also will need a separate card for CVS Caremark for your prescription drugs.  <\/p>\n<p>Please go to the University of Nebraska Benefits Website at <a href=\"http:\/\/www.nebraska.edu\/benefits\" target=\"_blanK\" rel=\"noopener\">www.nebraska.edu\/benefits<\/a> to download and print your paper CVS Caremark prescription drug card.   <\/p>\n<p>To find the prescription card:<br \/>\n<UL><LI>Click on Benefits Forms (listed under Other Benefits Information);<br \/>\n<LI>Then look under Prescription Drug Program and click on Caremark Identification Card.<\/UL><\/p>\n<p>Your CVS Caremark ID number is a 9-digit number and consists of your personnel ID number preceded by zeroes:<br \/>\n<UL><LI>If your personal ID number is five digits &#8212;
