The program focuses on the six dimensions of wellness:<br \/>\n<UL><LI>Social;<br \/>\n<LI>Physical;<br \/>\n<LI>Intellectual;<br \/>\n<LI>Spiritual;<br \/>\n<LI>Emotional; and<br \/>\n<LI>vocational.<\/UL><\/p>\n<p><B>Patient perks<\/B><\/p>\n<p>Exercise is Medicine is a national initiative by the American College of Sports Medicine that encourages health care providers to assess patient&#8217;s exercise habits as a vital sign at each patient visit and recommend each patient to be physically active. <\/p>\n<p>Providers are encouraged to refer their patients 50 years of age or older to UNMC&#8217;s EngAge Wellness &#8220;Exercise is Medicine&#8221; provider referral program to receive:<br \/>\n<UL><LI>Free two week participation in the EngAge Wellness program;<br \/>\n<LI>A functional fitness assessment;<br \/>\n<LI>An individualized fitness program designed by a degreed exercise science professional;<br \/>\n<LI>One-on-one orientation to their fitness program;<br \/>\n<LI>An option to participate in group movement classes;<br \/>\n<LI>A referral to other appropriate services to reach patient wellness goals; and<br \/>\n<LI>Communication back to health care provider regarding patient&#8217;s participation and progress.<\/UL><\/p>\n<p><B>Services for providers<\/B><\/p>\n<p>Providers would receive communication regarding each patient&#8217;s participation and progress through the free trial period, and beyond, should the patient decide to join the Exercise is Medicine program.<\/p>\n<p><B>Program contacts<\/B><\/p>\n<p>Contact EngAge
