{"id":46948,"date":"2001-10-23T00:00:00","date_gmt":"2001-10-23T05:00:00","guid":{"rendered":"https:\/\/www.unmc.edu\/newsroom\/2001\/10\/23\/researchers-study-new-approach-to-preventing-treatingosteoporosis-in-breast-cancer-survivors\/"},"modified":"2022-08-29T10:51:09","modified_gmt":"2022-08-29T15:51:09","slug":"researchers-study-new-approach-to-preventing-treatingosteoporosis-in-breast-cancer-survivors","status":"publish","type":"post","link":"https:\/\/www.unmc.edu\/newsroom\/2001\/10\/23\/researchers-study-new-approach-to-preventing-treatingosteoporosis-in-breast-cancer-survivors\/","title":{"rendered":"Researchers Study New Approach to Preventing, Treating\n\nOsteoporosis In Breast Cancer Survivors"},"content":{"rendered":"<div class=\"panel body-content\"><div class=\"panel__container\"><p>Researchers at the University of Nebraska Medical Center College of<\/p>\n<p>Nursing are involved in a study to evaluate a new approach in preventing<\/p>\n<p>and treating osteoporosis in breast cancer survivors.<\/p>\n<p>As a result of chemotherapy, at least 50 percent of women who are diagnosed<\/p>\n<p>with breast cancer under age 50 experience premature ovarian failure that<\/p>\n<p>leads to early menopause. Because estrogen influences growth of breast<\/p>\n<p>cancer in women with estrogen receptor positive cancers, women with a history<\/p>\n<p>of breast cancer are often not candidates for hormone replacement therapy,<\/p>\n<p>the most effective preventive treatment for osteoporosis.<\/p>\n<p>Osteoporosis causes deterioration of bone tissue, which leads to bone<\/p>\n<p>fragility and an increased susceptibility to fractures of the hip, spine<\/p>\n<p>and wrist, according to the National Osteoporosis Foundation. The disease<\/p>\n<p>is a major public health threat for more than 28 million Americans, 80<\/p>\n<p>percent of whom are women. An average of 24 percent of hip fracture patients<\/p>\n<p>age 50 and over die in the year following their fracture. Caucasian and<\/p>\n<p>Asian women are at increased risk for osteoporosis.<\/p>\n<p>This is a&nbsp; really significant problem. The good news is that more<\/p>\n<p>women are surviving breast cancer than ever before, said Nancy Waltman,<\/p>\n<p>Ph.D., associate professor of nursing, UNMC College of Nursing Lincoln<\/p>\n<p>Division and principal investigator of the study. Unfortunately, they<\/p>\n<p>have another concern &#8212; they may be at greater risk of osteoporosis than<\/p>\n<p>healthy women, and the onset of osteoporosis may occur at an earlier age.<\/p>\n<p>The purpose of the one-year study is to evaluate the feasibility and<\/p>\n<p>effectiveness of a multi-component intervention in post-menopausal women<\/p>\n<p>between the ages of 40 and 65 years, who have completed treatment for early<\/p>\n<p>stage breast cancer and who cannot receive hormone replacement therapy<\/p>\n<p>(HRT). Specific objectives of the pilot study are to determine adherence<\/p>\n<p>of the intervention, and to look at differences between the groups such<\/p>\n<p>as quality of life and vigor, and outcomes such as bone mineral density,<\/p>\n<p>muscle strength and activity level.<\/p>\n<p>Women in Omaha, Kearney and Scottsbluff will participate in the study.<\/p>\n<p>The study is funded by the Regional West Foundation of Scottsbluff,<\/p>\n<p>Neb., and national organizations: Oncology Nursing Foundation; Sigma Theta<\/p>\n<p>Tau International Honor Society of Nursing; and the American Nurses Foundation.<\/p>\n<p>The study will evaluate a group of 30 women who will taking calcium<\/p>\n<p>with vitamin D, and Fosamax&reg;, a drug shown to rebuild bone mass. Women<\/p>\n<p>also participate in strength and weight training exercises and receive<\/p>\n<p>education on osteoporosis as well as strategies to promote adherence to<\/p>\n<p>the intervention. A control group of seven to 12 women will follow the<\/p>\n<p>same protocol but without strength and weight training.<\/p>\n<p>Eligible study participants will receive evaluations, including muscle<\/p>\n<p>strength and balance tests and bone mineral density screenings using Dual-Energy<\/p>\n<p>X-ray Absorptiometry (DEXA). DEXA screening is painless, takes less than<\/p>\n<p>15 minutes and doesnt require clothes removal. Women will be evaluated<\/p>\n<p>at various intervals during the study.<\/p>\n<p>Researchers say the study is unique because it combines exercise with<\/p>\n<p>other components used in the fight against osteoporosis &#8212; calcium, vitamin<\/p>\n<p>D and drugs that build bone mass.<\/p>\n<p>We want to know how much exercise improves on the effects of the medication,<\/p>\n<p>said Janice Twiss, Ph.D., UNMC associate professor of nursing and womens<\/p>\n<p>health nurse practitioner and co-principal investigator of the study. Dr.<\/p>\n<p>Twiss said a strength-weight training program will be used to strengthen<\/p>\n<p>muscle and bone mass in the hip, spine and wrist.<\/p>\n<p>After age 65, women who arent on HRT are at great risk for osteoporotic<\/p>\n<p>fractures, said Twiss said. Women whove had breast cancer and have gone<\/p>\n<p>through chemotherapy have a longer period of time where bone is lost.<\/p>\n<p>She said there is a growing concern in regards to osteoporosis as people<\/p>\n<p>live longer. The estimated annual cost of osteoporosis is $15 to 20 million<\/p>\n<p>annually.<\/p>\n<p>Its a more expensive chronic illness than congestive heart failure<\/p>\n<p>and asthma, Dr. Twiss said. Women can live a long time, but theyre disabled.<\/p>\n<p>They end up in a nursing home.<\/p>\n<p>She said women get fractures in their spine simply by reaching up to<\/p>\n<p>get something, sneezing or walking down steps. Most often, hip fractures<\/p>\n<p>usually occur with a fall.<\/p>\n<p>An important part of the study evaluates study participants adherence<\/p>\n<p>with components of the study. Weve recognized that adherence not only<\/p>\n<p>to exercise, but to medications, is a problem. So weve included strategies<\/p>\n<p>to improve adherence, Dr. Waltman said.<\/p>\n<p>Information gathered from this study will be used for a larger study.<\/p>\n<p>In June, Drs. Waltman and Twiss published baseline results of bone mineral<\/p>\n<p>density in postmenopausal breast cancer survivors in the Journal of the<\/p>\n<p>American Academy of Nurse Practitioners. They were surprised to find 80<\/p>\n<p>percent of the women had signs of osteopenia or osteoporosis and about<\/p>\n<p>45 percent were not aware they had a problem because theyd never been<\/p>\n<p>told.<\/p>\n<p>Very few women had bone mineral density measurements before the study,<\/p>\n<p>Dr. Waltman said. Many of these women didnt even realize that osteoporosis<\/p>\n<p>was something they should be concerned about it.
