{"id":47014,"date":"2000-02-28T00:00:00","date_gmt":"2000-02-28T06:00:00","guid":{"rendered":"https:\/\/www.unmc.edu\/newsroom\/2000\/02\/28\/unmcs-dr-gay-canaris-co-authors-study-in-archivesof-internal-medicine\/"},"modified":"2022-08-29T10:55:56","modified_gmt":"2022-08-29T15:55:56","slug":"unmcs-dr-gay-canaris-co-authors-study-in-archivesof-internal-medicine","status":"publish","type":"post","link":"https:\/\/www.unmc.edu\/newsroom\/2000\/02\/28\/unmcs-dr-gay-canaris-co-authors-study-in-archivesof-internal-medicine\/","title":{"rendered":"UNMC&#8217;s Dr. Gay Canaris Co-Authors Study in Archives\n\nof Internal Medicine:"},"content":{"rendered":"<div class=\"panel body-content\"><div class=\"panel__container\"><p><\/FONT><\/I><\/p>\n<p><BR><B><FONT SIZE=+3>Study Calls for Expanded Testing As Undiagnosed Thyroid<\/p>\n<p>Disease May Affect Thousands<\/FONT><\/B><\/p>\n<p><P>The largest study to date evaluating the prevalence of thyroid disease<\/p>\n<p>indicates there may be more than 13 million Americans who are unaware they<\/p>\n<p>have a thyroid condition even though the disease may be impacting their<\/p>\n<p>short- and long-term health.&nbsp; This is double the previously suspected<\/p>\n<p>number of undiagnosed cases in the U.S., according to data published in<\/p>\n<p>the Feb. 28 issue of Archives of Internal Medicine.<\/p>\n<p><P>As thyroid function declined, the study found, patients reported more<\/p>\n<p>symptoms.&nbsp; But the relationship between the symptoms reported and<\/p>\n<p>progressive thyroid failure is not as clear as would be expected, nor is<\/p>\n<p>any single symptom a definitive indicator of thyroid failure. While there<\/p>\n<p>was an increase in the likelihood of thyroid disease as the number of reported<\/p>\n<p>symptoms increased, these symptoms are often vague and develop slowly so<\/p>\n<p>they may go unnoticed.<\/p>\n<p><P>Thyroid symptoms are so common and are often mistaken for signs of<\/p>\n<p>aging, menopause, depression or stress, said Gay Canaris, M.D., assistant<\/p>\n<p>professor in the University of Nebraska Medical Center Department of Internal<\/p>\n<p>Medicine and lead author of the study. Since we cant rely upon reported<\/p>\n<p>symptoms alone to detect disease, we as physicians should be conducting<\/p>\n<p>thyroid testing more often.<\/p>\n<p><P>The thyroid gland plays a vital role in overall body function during<\/p>\n<p>all stages of life. Although relatively small, it produces a hormone that<\/p>\n<p>influences every cell, tissue and organ in the body.&nbsp; The thyroid<\/p>\n<p>regulates the bodys metabolism  the rate at which the body produces energy<\/p>\n<p>from nutrients  and affects heart rate, energy and mood. Thyroid disease<\/p>\n<p>can be diagnosed through a simple blood test called a TSH (thyroid stimulating<\/p>\n<p>hormone).<\/p>\n<p><P>When the thyroid gland is not working properly, it can become either<\/p>\n<p>underactive (resulting in hypothyroidism) or overactive (resulting in hyperthyroidism).&nbsp;<\/p>\n<p>Signs and symptoms of an underactive thyroid include fatigue, depression,<\/p>\n<p>forgetfulness, unexplained weight gain, and menstrual irregularities.&nbsp;<\/p>\n<p>An overactive thyroid is marked by irritability\/nervousness, sleep disturbances,<\/p>\n<p>unexplained weight loss, muscle weakness and vision problems.&nbsp; If<\/p>\n<p>left untreated, thyroid disease may lead to an increased risk for heart<\/p>\n<p>disease, osteoporosis and infertility.<\/p>\n<p><BR>&nbsp;<\/p>\n<p><BR>Thyroid disease can strike anyone at any time, but is more common<\/p>\n<p>in women, Dr. Canaris said.<\/p>\n<p><P>One woman in eight will develop a thyroid disorder during her lifetime,<\/p>\n<p>Dr. Canaris said.&nbsp; Incidence also increases with age.&nbsp; By age<\/p>\n<p>60, more than 20 percent of American women will have a thyroid disorder.<\/p>\n<p><P>The study, one of the largest patient populations ever evaluated, found<\/p>\n<p>that of the 25,862 participants, 11.7 percent had abnormal serum TSH levels.&nbsp;<\/p>\n<p>Evaluating incidence according to over- and underactive thyroid cases,<\/p>\n<p>2,450 patients or 9.5% of the population had an underactive thyroid (hypothyroidism)<\/p>\n<p>and 570 or 2.2 percent of the population had an overactive thyroid (hyperthyroidism).<\/p>\n<p><P>The study also found that even the slightest decrease in thyroid function<\/p>\n<p>may increase cholesterol levels, possibly increasing a patients risk for<\/p>\n<p>cardiovascular disease.&nbsp; This link between the early stages of underactive<\/p>\n<p>thyroid and cholesterol levels provides evidence that untreated patients<\/p>\n<p>may experience serious negative health consequences.<\/p>\n<p><P>While it has been known for decades that overt hypothyroidism contributes<\/p>\n<p>to elevated cholesterol levels, this is the largest study to show that<\/p>\n<p>the cholesterol levels among patients with mildly decreased thyroid function<\/p>\n<p>were significantly higher than the cholesterol levels in people with normal<\/p>\n<p>thyroid function.<\/p>\n<p><P>&nbsp;This study was novel in that it drew a clearer connection between<\/p>\n<p>mild or early stages of thyroid failure and its effect on cholesterol levels,<\/p>\n<p>said Dr. E. Chester Ridgway, head of the Division of Endocrinology, University<\/p>\n<p>of Colorado, and senior author of the study.&nbsp; It showed that as the<\/p>\n<p>thyroid gland fails and less thyroid hormone is produced, blood cholesterol<\/p>\n<p>levels rise.&nbsp; This has serious long-term consequences for the patients<\/p>\n<p>health particularly in the area of cardiovascular disease.<\/p>\n<p><P>This study clearly offers a great opportunity for preventative medicine,<\/p>\n<p>especially for Americas women, Dr. Canaris said. Once diagnosed, hypothyroidism<\/p>\n<p>can be treated with a synthetic hormone replacement tablet, levothyroxine<\/p>\n<p>sodium, taken once a day. All we have to do is broaden the scope of current<\/p>\n<p>thyroid testing, and we can help improve the quality of life for thousands<\/p>\n<p>of people.<\/p>\n<p><P>UNMC is the only public academic to UNMC scientists, and UNMCs funding from the National Institutes<\/p>\n<p>of Health increased by 28 percent, going from $16.2 million to $20.7 million.<\/p>\n<p>UNMCs educational programs are responsible for training more health professionals<\/p>\n<p>practicing in Nebraska than any other
