Dr. Imran Alam Brings Advanced Surgical Monitoring<\/p>\n

System to UNMC\/NHS<\/p>\n

All surgeons rely upon complex systems to monitor what is beyond the<\/p>\n

structures and tissues immediately in front of them, and physicians who<\/p>\n

operate on the brain and spinal cord are especially meticulous when monitoring<\/p>\n

a patient.  Imran Alam, M.D., a new physician at the University of<\/p>\n

Nebraska Medical Center and the Nebraska Health System, is one of the countrys<\/p>\n

highest trained neuromonitors. He joined UNMC\/NHS after more than five<\/p>\n

years at Columbia University.<\/p>\n

Neuromonitoring is an early warning system for the operating surgeon,<\/p>\n

and in fact, is not limited to just neurosurgery, said Dr. Alam, who is<\/p>\n

a surgical neurophysiologist and assistant professor in the department<\/p>\n

of  surgery.  We want the earliest possible detection of any<\/p>\n

significant change in nerve activity or reduction in blood flow, or any<\/p>\n

potential trauma to electrical contact points that control the five senses,<\/p>\n

memory, speech and muscle responses.<\/p>\n

Regardless of whether the surgery involves the brain, heart, eyes,<\/p>\n

ears or vascular system, there are many elements of human tissue that cant<\/p>\n

be scraped, bumped, compressed or otherwise injured without collateral<\/p>\n

damage to the patient damage that often isnt evident until after the<\/p>\n

operation.<\/p>\n

Lyal Leibrock, M.D., professor and chief, section of neurosurgery, said<\/p>\n

that surgeons who perform spinal instrumentations, skull base procedures<\/p>\n

and operate on spinal cord lesions have particular monitoring needs to<\/p>\n

insure that there is no injury caused to the spinal cord.<\/p>\n

Dr. Alam  is a significant addition to UNMC and is especially<\/p>\n

well-trained to help the section of neursurgery, orthopedics and the skull<\/p>\n

base program, Dr. Leibrock said.  His monitoring prevents neurologic<\/p>\n

injury to patients undergoing these procedures.<\/p>\n

In addition to providing surgical monitoring, Dr. Alam often provides<\/p>\n

pre-operation maps to guide surgical teams throughout certain procedures,<\/p>\n

especially in the brain.<\/p>\n

The monitoring system managed by Dr. Alam will be used in more than<\/p>\n

60 procedures involving neurosurgery, orthopedic surgery, vascular surgery,<\/p>\n

cardiovascular surgery, ear\/nose\/throat surgery and interventional radiology.<\/p>\n

There currently is no other physician in the Lincoln\/Omaha-area trained<\/p>\n

to monitor such an extensive list of procedures. He will be involved in<\/p>\n

surgeries at NHS University and NHS Clarkson hospitals.<\/p>\n

Some of the critical areas Dr. Alam monitors with highly sensitive devices<\/p>\n

are:<\/p>\n

· Any impact of the brain stem and nerves responsible for hearing<\/p>\n

and other vital functions.<\/p>\n

· Crucial brain activity during surgery involving epilepsy and<\/p>\n

cerebral aneurysms.<\/p>\n

· Any distortion in blood flow, vascular anatomy and indication<\/p>\n

of blood clots.<\/p>\n

· The motor and sensory pathways during spinal surgeries.<\/p>\n

This sort of monitoring is cost-effective, because it clearly outweighs<\/p>\n

the potential cost of post-operative medical and rehabilitative care for<\/p>\n

patients who suffer neurological damage during surgery, Dr. Alam said.<\/p>\n

Neuromonitoring is advantageous to the patient, surgeon, hospital, and<\/p>\n

insurance carrier, whenever there is a risk of physician-induced neurological<\/p>\n

injury.<\/p>\n
