{"id":47568,"date":"2003-08-14T00:00:00","date_gmt":"2003-08-14T05:00:00","guid":{"rendered":"https:\/\/www.unmc.edu\/newsroom\/2003\/08\/14\/unmc-college-of-nursing-new-specialty-prepares-nursesfor-dual-role-in-family-mental-health-will-help-ease-health-professionalshortage\/"},"modified":"2022-08-29T10:43:47","modified_gmt":"2022-08-29T15:43:47","slug":"unmc-college-of-nursing-new-specialty-prepares-nursesfor-dual-role-in-family-mental-health-will-help-ease-health-professionalshortage","status":"publish","type":"post","link":"https:\/\/www.unmc.edu\/newsroom\/2003\/08\/14\/unmc-college-of-nursing-new-specialty-prepares-nursesfor-dual-role-in-family-mental-health-will-help-ease-health-professionalshortage\/","title":{"rendered":"UNMC College of Nursing new specialty prepares nurses\n\nfor dual role in family, mental health; will help ease health professional\n\nshortage"},"content":{"rendered":"<div class=\"panel body-content\"><div class=\"panel__container\"><p>The University of Nebraska Medical Center College of Nursing has received<\/p>\n<p>a $750,000 federal grant that will help ease Nebraskas shortage of rural<\/p>\n<p>health professionals by preparing students in the dual capacity of a family<\/p>\n<p>nurse practitioner and advanced practice psychiatric\/mental health nurse.<\/p>\n<p>The U.S. Health and Human Services Bureau of Health Professions Division<\/p>\n<p>of Nursing is funding the grant.<\/p>\n<p>There are many vulnerable populations of rural and urban Nebraskans<\/p>\n<p>who dont have access to mental health services and whose physical and<\/p>\n<p>mental health care needs are not met, said Kate Fiandt, D.N.S., UNMC College<\/p>\n<p>of Nursing associate professor of nursing and project director of the grant.<\/p>\n<p>In Nebraskas rural areas, theres a huge lack of qualified mental health<\/p>\n<p>practitioners who can make a diagnosis and prescribe medications.<\/p>\n<p>Dr. Fiandt said many rural communities cant support a full-time psychiatric\/mental<\/p>\n<p>health advanced practice nurse, but one with both physical and mental health<\/p>\n<p>skills is an asset.<\/p>\n<p>This practitioner will be a valuable resource in Nebraska where 75<\/p>\n<p>percent of rural counties are federally designated mental health professional<\/p>\n<p>shortage areas and 36 percent of rural counties are federally designated<\/p>\n<p>primary care health professional shortage areas, Dr. Fiandt said. Though<\/p>\n<p>there are some dual-certified advanced practice nurses with this specialty,<\/p>\n<p>this may be the first one in the country that prepares the nurse for certification<\/p>\n<p>in both areas at the same time. The state very badly needs people skilled<\/p>\n<p>in both.<\/p>\n<p>Dr. Fiandt, who has been a family nurse practitioner for 27 years, said<\/p>\n<p>in some clinics, 80 percent of the people who seek family or primary care<\/p>\n<p>services have both physical and mental health needs.<\/p>\n<p>Mental problems get in the way of physical health, and poor, unmanaged<\/p>\n<p>health causes mental health problems, she said. If you treat depression,<\/p>\n<p>diabetes gets better. Various health conditions like diabetes and cancer<\/p>\n<p>can cause depression. This takes two different programs and streamlines<\/p>\n<p>them into one to enable a practitioner to treat both conditions.<\/p>\n<p>The specialty will prepare students as advanced practice nurses. Advance<\/p>\n<p>practice nurses are those with masters degrees and advanced clinical experience<\/p>\n<p>who diagnose, treat and manage illness, including nurse practitioners,<\/p>\n<p>clinical nurse specialists, nurse midwives, nurse anesthetists and other<\/p>\n<p>specialties. Some also prescribe medication.<\/p>\n<p>&nbsp;<\/p>\n<p>This fall, the college will accept seven full-time students with bachelors<\/p>\n<p>degrees in nursing. Students will spend the first year in academics. They<\/p>\n<p>will use distance education technologies such as the Internet, CD-ROMs<\/p>\n<p>and conference calls, and occasionally will be required to drive to UNMC<\/p>\n<p>College of Nursing sites in Omaha, Kearney or Scottsbluff. Along with classroom<\/p>\n<p>content during the second and third years of the program, students receive<\/p>\n<p>850 clinical hours of experience under the supervision of a preceptor in<\/p>\n<p>or nearby their communities.<\/p>\n<p>Dr. Fiandt said another advantage of the new specialty is that people<\/p>\n<p>can go to a health professional for their mental health needs without the<\/p>\n<p>stigma.<\/p>\n<p>In rural communities, many times people wont go to a psychiatrist.<\/p>\n<p>If they could go to see a primary care professional who is skilled in managing<\/p>\n<p>psychological problems, then it protects them from this stigma, Dr. Fiandt<\/p>\n<p>said.<\/p>\n<p>The grant also supports research on the role and integration of the<\/p>\n<p>two specialties.<\/p>\n<p>The importance and priority of such programs is evident in a recent<\/p>\n<p>report by the President&#8217;s New Freedom Commission on Mental Health (available<\/p>\n<p>at www.MentalHealthCommission.gov). In 86 pages, the authors discuss the<\/p>\n<p>failings of the current system and describe six major goals for transformation.<\/p>\n<p>The report estimates that 5 to 7 percent of adults have a serious mental<\/p>\n<p>illness. In children, about 5 to 9 percent have a serious emotional disturbance.<\/p>\n<p>The commission believes that Americans must come to understand that<\/p>\n<p>mental health is integral to their overall health, and recommends that<\/p>\n<p>mental illnesses be addressed with the same urgency as other medical problems.<\/p>\n<p>The commission also said stigma attached to mental illness must be eliminated<\/p>\n<p>because it discourages people from seeking care.<\/p>\n<p>Minorities and people in rural areas, the commission finds, have worse<\/p>\n<p>access to care, and often receive services that are not responsive to their<\/p>\n<p>needs. As a result, the burden of mental illness is heavier for these individuals.<\/p>\n<p>Dr. Fiandt said another example of how the practitioner could help is<\/p>\n<p>provide early intervention for families with children who have behavioral<\/p>\n<p>problems such as discipline and control issues. This practitioner could<\/p>\n<p>work with the family and get them back in charge of the situation, Dr.<\/p>\n<p>Fiandt said.
