Funded by Blue Cross Blue Shield of Nebraska with the primary goal to reduce hospitalization and emergency room visits made by high-risk patients with two or more chronic diseases, while improving the outcomes of their disease management.<\/li>\n<li>\nGreater Plains Collaborative (GPC) Clinical Data Research Network &ndash; Co-investigator James Campbell, M.D., professor. A new network of 10 leading medical centers in seven states committed to a shared vision of improving health care delivery through ongoing learning, adoption of evidence-based practices and active research dissemination.
