in Chronic Care Management (CENTRIC) funded two pilot projects last month that will support self-management of chronic conditions. <\/p>\n<p><TABLE border=\"0\" width=\"250\" cellspacing=\"10\" \/>\n<font face=\"arial\" size=\"1\"><br \/>\nMyra Schmaderer, Ph.D.<br \/>\n<\/FONT><br \/>\n<\/TD><br \/>\n<\/TR><br \/>\n<\/TABLE>Patrik Johansson, M.D., from the College of Public Health, in collaboration with Paula Schulz, Ph.D., from the College of Nursing, received a $50,000 grant titled &#8220;Cardiovascular Disease Self-Management in Rural Adults: A Pilot Intervention Study Using Technology and RN\/Community Health Worker Teams.&#8221; <\/p>\n<p>Myra Schmaderer, Ph.D., from the College of Nursing, in collaboration with Rachel Thompson, M.D., from the College of Medicine received a $40,000 grant titled &#8220;Self-Management in Heat Failure: M-Health Intervention.&#8221;<\/p>\n<p>CENTRIC funds interventions for use in varied clinical and community settings that test the efficacy or effectiveness of self-management interventions for chronic conditions to reduce burden and disability, improve functioning and health-related quality of life, strengthen patient
