The next appointments will be held May 2020. Check with your manager if you are not sure if you are required to be fit tested. <\/p>\n<p>The fit testing will occur during these dates at the following locations:<\/p>\n<ul>\n<li> Feb. 17-21: Nebraska Medical Center, Learning Resource Center 1, basement level of Storz Pavilion\n<li>Feb. 24-25: Nebraska Medical Center, Learning Resource Center 1, basement level of Storz Pavilion\n<li>Feb. 26: Bellevue Medical Center, Conference Room D\n<li>Feb. 27: Bellevue Medical Center, Conference Room C\n<li>Feb. 28: Internal Medical Associates in Grand Island<\/ul>\n<p><a href=\"https:\/\/ssl.nebraskamed.com\/Apollo\/\">Sign up in Apollo<\/a> for your appointment by logging in, selecting &#8220;Search for Trainings&#8221; on the left-hand side, then typing &#8220;fit testing.&#8221;<strong> Plan to arrive 10 minutes before your appointment. If you are late, you may be asked to reschedule. <\/strong><\/p>\n<p>Fill out and sign the applicable FROST form and bring it to your appointment<\/p>\n<ul>\n<li>If you&#8217;ve previously been fit tested by FROST: <a href=\"https:\/\/now.nebraskamed.com\/wp-content\/uploads\/2018\/07\/MEDICAL-HISTORY-REVIEW3.pdf\">Medical History Review<\/a>\n<li>If you&#8217;ve not previously been fit tested by FROST: <a href=\"https:\/\/now.nebraskamed.com\/wp-content\/uploads\/2017\/04\/OSHA-Med.-Quest.-1.pdf\">Respiratory Protection Medical Form<\/a><\/ul>\n<p>Fill out and sign the <a href=\"https:\/\/now.nebraskamed.com\/wp-content\/uploads\/2019\/10\/Consent-for-Treatment.pdf\">Consent for Treatment Form<\/a> and bring it to your appointment<\/p>\n<p>For UNMC employees only, in addition to Consent to Treatment, fill and sign the following forms, and bring it to your appointment<\/p>\n<ul>\n<li><a href=\"https:\/\/now.nebraskamed.com\/wp-content\/uploads\/2018\/07\/Nebraska-Medicine-Employee-Health-Record-Authorization-Rev.7.6.18.pdf\">Employee Health Authorization Form<\/a>\n<li><a href=\"https:\/\/now.nebraskamed.com\/wp-content\/uploads\/2019\/10\/Nebraska-Medicine-Employee-Health-Immunization-Allergy-Verification-Authorization-Rev.2.11.19.pdf\"> Immunization and Allergy Verification Form<\/a><\/ul>\n<p>Other notes:<\/p>\n<ul>\n<li> Registration will be closed 24 hours prior to each session, so be sure your appointment is in Apollo.
