Adult Gerontology Primary Care NP

Overview

The Adult-Gerontology Primary Care Nurse Practitioner AG-PCNP focuses on the diagnostic reasoning, assessment, and management of the primary health care of adolescents through the elderly. The AG-PCNP is prepared to provide preventive, chronic, and acute care. The main emphasis of the AG-PCNP role is on comprehensive and continuous care characterized by a long-term relationship with the patient. The role encompasses complex medical management, vulnerable population advocacy, and interprofessional team leadership to promote safe transitional care in and across settings.

Career opportunities includes college-based health clinics, comprehensive geriatric assessment centers, community-based care centers, palliative and hospice care, long-term care, and practice in specialty clinics.
