If you have a temporary or permanent physical, psychological, sensory, or learning disability, you may be eligible for reasonable accommodations. To begin the process of establishing a disability accommodation plan, documentation of your disability is required. Documentation may be in the form of:
- A letter from a medical doctor or other health care provider;
- Individual Education Plan (IEP) with Multidisciplinary Team Report (MDT);
- 504 Plan; or,
- Your current treating provider can complete a Provider Evaluation Form.
Any form of documentation you provide must include your diagnosis, functional limitations, and recommendations for accommodation. Documentation can be emailed or faxed to our office.
Please note that accommodations are not retroactive, so any accommodation plan established now would be applicable on a going-forward basis only.
Alternate Format Materials
For students who qualify for "E-text" as part of their accommodation plan, please contact ASC for guidance on alternate format materials and for assistance in setting up an individual BookShare account.
Accessible parking stalls are available throughout campus. To use these stalls, both a campus parking pass and a DMV-issued disabled parking permit are required.
