This interactive conference aims to strengthen interdisciplinary collaboration across the stroke recovery continuum, from emergency care to community reintegration. Participants will explore evidence-based strategies to streamline care transitions and minimize fragmentation. Through guided discussions and practical exercises, attendees will co-develop actionable tools and protocols that health care professionals can apply in clinical settings to enhance patient outcomes and reduce rehospitalization rates.
At the conclusion of this activity, participants should be better able to:
- Demonstrate interdisciplinary collaboration to improve continuity of care across the stroke recovery continuum.
- Identify evidence-based strategies for streamlining stroke transitions from emergency services to community reintegration.
- Develop actionable tools and protocols that can be implemented in clinical practice to optimize patient outcomes and reduce rehospitalization rates.
In support of improving patient care, this activity has been planned and implemented by University of Nebraska Medical Center and Nebraska Department of Health and Human Services. University of Nebraska Medical Center is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
Physicians
The University of Nebraska Medical Center designates this live activity for a maximum of 6.75 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Nurses
The University of Nebraska Medical Center designates this activity for 6.75 ANCC contact hours. Nurses should only claim credit for the actual time spent participating in the activity.
