Report a Safety Concern

Request a Pickup

We provide pick-ups for batteries, chemical and radiation waste. Please complete all required information before submitting one of the linked forms. Allow at least five working days from date of submission for items to be collected. Some request forms include links to fact sheets with more information. With questions or for assistance with a request form, call 402-559-6356.

Incident Reporting

UNMC and Nebraska Medicine employees, students and volunteers are required to report accidents and incidents involving injury. Forms also are provided for near-misses, unsafe conditions and compliance issues.
