Collaboration And Proactive Teamwork Used to Reduce Falls

The Collaboration and Proactive Teamwork Used to REduce – “CAPTURE” – Falls Program is a quality improvement program to support the implementation of evidence-based fall risk reduction practices using an interprofessional team approach to reduce the risk of inpatient falls in Critical Access Hospitals. Since 2012, we have assisted over 30 Nebraska hospitals, the majority being Critical Access Hospitals, to develop and implement a customized action plan to improve the structures and processes for fall risk reduction in their facilities.

In 2021, we established a relationship as an analytical subcontractor with the Nebraska Coalition for Patient Safety, which is designated by the Agency for Healthcare Research and Quality as a federally listed Patient Safety Organization.

Our work, including the creation of this website and related resources, has been supported by grant numbers R18HS021429 (August 2012-July 2015) and R03HS024639 (April 2016-March 2017) from the Agency for Healthcare Research and Quality. Additional support is provided by the Nebraska Department of Health and Human Services Office of Rural Health through the Medicare Rural Hospital Flexibility Program (January 2015-present).
