CAPTURE Falls Action Plan: The CAPTURE Falls Action Plan created by the UNMC CAPTURE Falls Team can be used to set priorities and document progress toward your fall risk reduction program goals.
Tell Your Falls Story: The CAPTURE Your Falls Story Flow Chart, created by the CAPTURE Falls team can be used to craft a persuasive message for audiences within your organization to support implementation of priorities indicated in your action plan.
AHRQ Action Plan: Tool 2F Action Plan in the AHRQ Fall Prevention Toolkit can provide you with a framework and series of steps to create and implement your fall risk reduction program improvement initiative. This action plan is populated with some improvement actions for consideration.
