Protecting Mothers After Childbirth: The STAMPP-HTN Program

The Systemic Treatment and Management of Postpartum Hypertension program, led by Dr. Sarosh Rana and directed by Dr. Rebecca Rimsza, is designed to improve maternal outcomes through structured remote blood pressure monitoring and coordinated postpartum care.
STAMPP is a postpartum blood pressure program designed to support timely identification, monitoring and management of postpartum hypertension while keeping patients safely at home whenever possible.
Program Goals
- Reduce complications related to postpartum hypertension
- Improve long-term cardiovascular health for mothers
- Provide consistent, evidence-based postpartum follow-up
- Support coordinated care between obstetrics and primary care
Patient Enrollment
- All patients with hypertensive disorders of pregnancy are automatically enrolled before discharge.
- Patients receive education on blood pressure monitoring and symptom awareness.
- Patients receive a Bluetooth-enabled blood pressure cuff.
Remote Blood Pressure Monitoring
- Patients measure their blood pressure twice daily.
- Readings are immediately uploaded and reviewed in real time by dedicated nurses with established pathways of care.
- High-risk blood pressure readings or symptom alerts are reviewed and escalated by the CareConnect nursing team according to established protocols. Providers are contacted only when a patient is instructed to return to the hospital or when additional medical judgment is required.
Clinical Monitoring and Escalation
- Protocol-driven escalation may include medication adjustment, clinic follow up or higher level evaluation by leveraging telehealth visits.
- All patients are continuously monitored through standardized protocols.
- Critical escalation triggers include:
- Severe-range blood pressure readings
- Symptoms concerning for severe features
- Patients are contacted promptly when thresholds are met.
Patients delivering at Nebraska Medicine who meet any of the following criteria are enrolled in STAMPP-HTN:
- Blood pressure of 140/90 mmHg or higher postpartum
- Diagnosis of chronic hypertensions, preeclampsia, or gestational hypertension
- Discharge on antihypertensive medication
Eligible patients are scheduled for follow up in the Postpartum Hypertension (PPHTN) Clinic.
