Residency Credentialing Requests

Effective January 1, 2017: Due to the increase in requests for information/verification of residency training at the UNMC Obstetrics and Gynecology Residency Program, the Division of Education now charges a fee for verifications. All form verifications will be subject to this charge. The verification methods include:

Private verification form ($50 fee)
Our written standard verification form with training dates only ($25 fee)
For verification via telephone of the training dates only there is no fee. Please call Jessica Wilde at 402-559-6160 to verify the dates over the phone.

Payment Methods and Request Form:

Credit card: Complete the Residency Verification Request Form and enter the credit card information.
Pay by check: Complete the Residency Verification Request Form and indicate the payment method as "pay with a check." Checks should be made payable to the UNMC Department of Obstetrics and Gynecology. Please mail the check and the name of the past resident to:
