The University of Nebraska Medical Center's Munroe-Meyer Institute for Genetics and Rehabilitation (MMI) provides a unique range of services for individuals with disabilities.

Most appointments are scheduled through our Patient Information Office.

Insurance

Before making an appointment it is a good idea for you to contact your insurance company or payer to verify coverage and confirm that Nebraska Medicine and Munroe-Meyer Institute providers are participants in their plan.

Appointment information

• Within the email, please provide the following information:
  
  • Patient's full name and date of birth
  • Provider's name or the department you're looking to schedule with
  • Reason for appointment
  • Your contact information (name and phone number and/or email address) and relation to the patient
• A patient information representative will contact you to obtain information needed to schedule your appointment.

Patient registration

Completed forms are often required prior to scheduling an appointment. Our intake specialists will assist you in knowing which forms you need for your appointment.

Referrals

An individual can be self-referred to the Munroe-Meyer Institute or referred by a physician or other health care professional, school district, social service agency or family member.
