Med center employees whose job description requires them to be fitted to an N95 respirator must complete an annual respirator fit test in 2023 per OSHA Standards.
The remaining 2023 N95 respirator fit testing sessions will be held at the following times/locations:
- Aug. 14 to 28: Nebraska Medical Center, basement level of Storz Pavilion, Learning Resource Center 1 (LRC-1)
- Aug. 29 and 30: Bellevue Medical Center, conference rooms A, B, and C
- Aug. 31 and Sept. 1: Village Pointe Health Center, 111 building, level two, conference room 1
- Dec. 4 to 15: Nebraska Medical Center, basement Level of Storz Pavilion, Learning Resource Center 1 (LRC-1)
Fit testing is offered to Nebraska Medicine and UNMC employees only. Contracted staff must continue completing any fit testing requirements through their contracting agency.
Nebraska Medicine and UNMC employees who work at Grand Island Health Center or University Health Center will have separate fit testing sessions from those listed above, which will be coordinated with the managers of those areas.
Registration is open for the Aug. 14 through Sept. 1 session. Follow these steps to register for an appointment through Apollo:
Step 1: Sign up in Apollo for your appointment
- Log in to Apollo
- Select “Search for Trainings” (on the left side)
- Type “Fit Testing” in the search bar
Drop-in appointments cannot be guaranteed and will be accommodated pending availability. Employees who arrive late for their fit testing appointment may be asked to reschedule.
Step 2: Complete the following paperwork to bring with you to your appointment:
Nebraska Medicine and dually employed employees
- Consent for Treatment Form
- FROST Respirator Medical Evaluation Questionnaire
- FROST Respirator Clearance Form
UNMC employees
- Consent for Treatment Form
- FROST Respirator Medical Evaluation Questionnaire
- FROST Respirator Clearance Form
- Employee Health Authorization Form
- Immunization and Allergy Verification Form
Employees who arrive at their fit testing appointment without completed paperwork may be asked to reschedule.
