The Munroe Meyer Institute Care Coordination Program is expanding to meet the growing needs of our patients and families at MMI. This program plays a vital role in connecting individuals to the care, services and community resources they need to thrive. As health care becomes more complex, our care coordination team ensures that no one feels lost navigating medical appointments, referrals from health care providers and support systems.
Recently, the program has grown to include additional team members, including an interpreter and social workers, to provide more comprehensive and inclusive support. Our interpreters help break down language barriers, ensuring that every patient and caregiver can effectively communicate with their specialty providers and fully understand their care plans. Social workers bring expertise in addressing social determinants of health, such as housing, transportation and financial challenges, that can affect a patient’s ability to stay connected and engaged in services.
Together, this multidisciplinary team works closely with patients, families and medical providers to create personalized care plans.
