NU Flex 2003 forms due Dec. 6

The NU Flex 2003 change forms are due in the UNMC Benefits Office (ZIP 5470) on Friday, Dec. 6. If you are not making any changes for 2003, and you are not participating in the reimbursement account, you need not return your form. If you wish to participate in the reimbursement account in 2003, you must return your NU Flex 2003 change form. The form must be notarized if you are signing up for the health care reimbursement account.
