CENTRIC seeking new members

The Center for Patient, Family & Community Engagement in Chronic Care Management (CENTRIC) is seeking new members.

CENTRIC, a center in the College of Nursing, promotes multidisciplinary teams to implement strategies that will promote self-management to maintain or improve patient’s health and prevent a decline in health in individuals with multiple concurrent chronic conditions.

More than one in four Americans have multiple concurrent chronic conditions (MCCs). Patients with MCCs have a poorer quality of life, psychological distress, worsening functional capacity, longer hospital stays, and more postoperative complications, leading to higher costs of care. Higher costs in patients with MCCs places a tremendous burden on the U.S. health care system, and health care is less costly for MCC patients who are proactive in their care.

Members of the UNMC CENTRIC Center have the opportunity to take advantage of benefits that include:

  • Opportunities to network and collaborate with colleagues across UNMC for research initiatives related to self-management in MCC;
  • Participation in a research interest group — “CENTRIC Chronic Care Self-Management Research Interest Group” — that meets bi-monthly;
  • Providing formative grant review for self-management in MCC studies;
  • Building research teams around chronic self-care management;
  • Applying for CENTRIC pilot grants;
  • Accessing or consultation regarding monitoring equipment, self-management tools, or measures acquired by the Technology Common Metric Committee (TCM);
  • Receiving the CENTRIC newsletter for updates on upcoming events and other opportunities for members; and
  • Membership registration benefits for sponsored CENTRIC events (e.g. reduced charge, no registration fee).

Join CENTRIC by completing the membership application.
