In the cards: Info on health benefits

As a reminder, all enrollees in the UNMC health insurance plan will need two different cards.

For new enrolleees, the card from Blue Cross Blue Shield should be arriving shortly. It will be pre-printed with your name and ID number. The names of your covered dependents will not be printed on the cards. If you need additional cards, please contact Blue Cross Blue Shield at 1-888-592-8963.

You also will need a separate card for your prescription drug coverage. The CVS Caremark card is a printable card and will not be mailed to you. To print your card, go to the University of Nebraska Benefits website, click on the “Benefits” Forms link listed under the “Other Benefits Information” header. Then look under “Prescription Drug Program,” and click on “Caremark Identification Card.”

Your CVS Caremark ID number is 9 digits, and consists of your personnel ID number preceded by zeroes, i.e. 000012345 or 000123456. Your personnel number can be found on your paystubs in Firefly as well as the confirmation statement you received during NUFlex 2014 open enrollment.
