Free mail-service generic drugs available for HRA participants

Those who completed the Wellstream/Health Risk Assessment (HRA) last fall and are, therefore, eligible for the free generic prescriptions through CVS Caremark using their mail service program, can now access that benefit.

Here’s how the CVS Caremark mail order Service Program Works:

  • Log in to the University’s Benefits Web site
  • On the right-hand side of the page click on: Benefit Forms –>
  • In middle of the page you will find: Prescription Drug Program –> Caremark Mail Order Request Envelope
  • Print this two page form, complete,and mail it to the CVS Caremark address below (address also is typed address in the upper right-hand corner of the first page of the form):

If you are accessing the program for the first time, you will need to enclose the original prescription(s) from your doctor for all requested medications and the required payment of the deductible and co-payment if filling a brand name drug (or give them credit card information).

If you are filling a generic, you will need to enclose a note that indicates that as of Jan. 1, no co-payment is required.

Again, this free generic benefit only pertains to those who completed the Wellstream/Health Risk Assessment during the benefits enrollment period of Nov. 16-Dec. 4.
