IRB to no longer accept old forms as of Sept. 1

As of Wednesday (Sept. 1), the Institutional Review Board will ONLY accept the recent versions of the applications and forms available on the IRB website.

Any submissions that are turned in on older forms/applications will be sent back, without regard to deadlines or approval status.
