Employees must complete a new online consent form before attending one of the free flu vaccination clinics, such as those scheduled for Wednesday from 7 to 9 a.m. and 1 to 3 p.m. in University Tower’s Private Dining Rooms A and B.
The badge number located on the back of your employee badge will be required to complete the consent form.
