Last shot for flu shots!

Consent form

Employees can complete a new online consent form before attending one of the free flu vaccination clinics.

The badge number on the back of your employee badge will be required to complete the consent form.

Visit http://newintranet.nebraskamed.com/vaccine/ to access the consent form or for more information.

Consent forms also are available at the clinics.

The final free flu shot clinic for employees will be held today from 7 to 9 a.m. in Storz Pavilion 7 and 8.

Staff must show employee identification badges to receive the free vaccinations. Vaccinations for all off-site locations will be scheduled by Employee Health. Off-site locations will be notified with clinic dates and times.

The live attenuated flu vaccine (LAIV, intranasal vaccine, FluMistTM) will not be stocked for routine administration to staff.

What about offering a flu shot clinic for those of us who work the night shift? It's very difficult to come in during the morning hours after working a long night shift. Please.
