Required Immunizations

All incoming, on-campus students must document receipt of immunizations prior to registration. Online students (except students whose training includes clinical settings and patient contact) are exempt.

Additional immunizations may be required before working in clinical or community partnerships. Please review immunization guidelines set by UNMC Student Health for additional immunization requirements and due dates.

Immunization Documentation

COVID-19 Vaccine

  • Documentation of vaccines

    OR
  • Declination of vaccine

Varicella (Chicken Pox)

  • Documentation of two (2) doses of varicella vaccine 

    OR
  • Evidence of immunity documented by a serologic blood test

Flu Vaccine

  • Documentation of current flu vaccine 

    OR
  • Declination of vaccine

Hepatitis B

  • Documentation of three (3) vaccine series and a positive Hepatitis B Surface Antibody Titer (required)
  • A positive titer alone will be accepted with lab report.

MMR (Measles/Mumps/Rubella)

  • Documentation of two (2) vaccines

    OR
  • Positive antibody titers (blood test) for all three diseases

Polio

  • Documentation of most current polio vaccine

    OR
  • If US born citizen, please complete the No Travel Consent.

Tuberculosis

  • Documentation of a two-step TB skin test (One test consists of a placement and a read. Two tests will need to be completed). 

    OR
  • Interferon Gamma Release Assay (IGRA blood test), Quantiferon (blood test) or T spot (blood test)

 

Required upon admission to program.

Tdap Tetanus/Diphtheria/Pertussis
  • Documentation of one (1) current dose of Tdap within the last ten (10) years

Immunization Center Guide for Students

Log in to MyRecords using your UNMC NetID and log in information. 

The screen displays a Message Center section that shows messages/notices sent to you as well as any To Do List items, including those from Student Health. Please note that a To Do List item is not a Hold and will not prevent you from registering. A Hold would prevent you from registering.

In the Immunization Center section, click Access Immunization Center. 

The Immunization Form appears on your screen. Click Immunization Form

On the Immunization Form Instructions screen, read and review the information. Then, click Next.

Review and complete sections 2 through 7 of the Immunization Form. Click Next to advance to each part; click Back to move to a previous section.

Read each section carefully and make accurate selections.

  • Choose the Vaccination Type or Titer Results type for your immunization
  • Enter the date of the immunization, and click Next.
  • Complete these fields for all remaining immunization section. Submit all immunization records simultaneously. Incomplete forms will not be accepted.
    • COVID-19 (Coronovirus)
    • Varicella (chicken pox)
    • Hepatitis B
    • Polio
    • PPD (Tuberculosis screening)
    • TDAP (Tetanus/Diptheria/Acellular Pertussis)

On the 8th screen, Attach Documentation, click Choose File to upload your immunization supporting documents.

Then, click Complete. Your information will be submitted to Student Health Administration for review.

If you have questions or receive any error messages, email Student Health Administration. In your message, include your NUID and a screenshot of the error message. Student Health Administration may also be reached by calling 402-559-5158.

Viewing/Printing Your Immunization Record

If you would like a printout of your UNMC Immunization Record, you may access it from the Immunization Center home screen. Click View/Print Immunization Record.
